If your car insurance provider requires proof of your student status, you can request an enrollment verification through the following methods:

Request Online

  • Log in to your Self-Service account
  • Submit a request for an enrollment verification

Request In Person

  • Visit the Student Records Office at:
    • Boone Campus, or
    • Edgewood Campus
  • Staff can complete your insurance verification form if needed
