How can I request a copy of my diploma and is there a fee?

Replacement credentials (diplomas/certificates) are available upon request from the Records Office either on campus or by sending a written statement that must include your name at the time of graduation, Student ID (if available), your date of birth, mailing address, and a contact phone number.

The replacement credential will take approximately 6 to 8 weeks to arrive. Please note that there is a fee for replacing the credential.

Replacement credentials display signatures of the current Jefferson Community & Technical College president, Board of Regents chair, KCTCS Chancellor, and KCTCS President and the original award date. There is a $20.00 charge for each replacement credential. Since credentials are not produced in house, it may take 10-12 weeks for processing & delivery. A separate application has to be completed for each degree/diploma to be replaced.

The form can be located at the Records Office Forms page (select Duplicate Credential Request) and turned into the Records Office or mailed to JCTC, Attn: Graduation, 109 East Broadway, Louisville, KY 40202. There is a $20.00 per each credential requested. Please only cash, money order or personal check.

*Paying with personal check will delay the processing by at least 3 weeks.
